Job OverviewProvides direct and indirect care in the inpatient rehabilitation setting. Communicates with physicians, Director of Nursing (DON), Therapy Department, and co-workers, as appropriate, about changes in patient's clinical condition, including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, performs general nursing duties and functions while under the supervision of the RN and within the scope of the LPN practice acts.
Responsibilities- Knowledgeable of medications and their correct administration based on the age of the patient and their clinical condition.
- Follows the five medication rights and reduces the potential for medication errors.
- Performs patient care responsibilities considering needs specific to the standard of care for patient's age.
- Demonstrates ability, under direct management of RN, to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes adult, geriatric, and the general patient population.
- Able to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Able to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan daily for effectiveness.
- Integrates the nursing plan of care with the Therapy Department treatment plan to attain the patient's optimal level of functioning.
- Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate.
- Demonstrates ability to perform treatments and provide services to the level of licensure.
- Communicates appropriately and clearly to team leader, co-worker, physicians, and the Therapy Department.
- Consults with therapy staff daily to provide for an interdisciplinary approach to the patient's needs and documents patient's status to include rehabilitation therapy progress.
- Provides care appropriate to condition and age of the patient, including adult, geriatric, and general population.
- Demonstrates an ability to assist physicians with procedures and performs services requiring technical and manual skills under the direction of an RN.
- Manages and operates equipment safely and correctly.
- Obtains intravenous access as needed and administers intravenous medications according to scope of practice.
- Performs venipuncture according to order to assist physicians with care and diagnosis.
- Performs skin/wound assessment every shift. Provides wound care as ordered by physician or other licensed provider.
