Licensed Practical Nurse (LPN) - Embrace a culture of healing and upliftment vides direct and indirect care in the inpatient rehabilitation setting. Communicates with physicians, director of nursing, therapy dept., and colleagues about changes in patient clinical condition and diagnostic studies. Responds quickly and accurately to changes in condition or response to treatment. Performs general nursing duties under RN supervision and within LPN practice scope.
Key Responsibilities- Knowledge of medications and correct administration based on patient age and clinical condition.
- Follows the five medication rights to reduce medication errors.
- Performs patient care considering needs specific to standard of care for patient age.
- Performs head‑to‑toe assessment on all patients and reassessments per policy under RN management.
- Assesses and reassesses pain, utilizes appropriate pain management techniques, and educates patients and families regarding pain management.
- Provides all aspects of patient care in an environment that optimizes patient safety and reduces medical/health‑care errors.
- Revises plan of care as indicated by patient response to treatment and evaluates overall plan daily for effectiveness.
- Integrates nursing plan of care with the therapy department treatment plan to attain optimal patient functioning.
- Forms teaching plan based on identified learning needs and evaluates effectiveness; includes family in teaching as appropriate.
- Performs treatments and provides services at a level of licensure.
- Communicates clearly to team leader, co‑worker, physicians, and therapy department.
- Consults with therapy staff daily to provide interdisciplinary approach to patient needs and documents patient status.
- Provides care appropriate to condition and age of patient, including adult, geriatric, and general population.
- Assists physicians with procedures and performs services requiring technical and manual skills under RN direction.
- Manages and operates equipment safely and correctly.
- Obtains intravenous access as needed and administers IV medications per scope of practice.
- Performs venipuncture as ordered to assist physicians with care and diagnosis.
- Performs skin/wound assessment every shift and provides wound care as ordered by physician or another licensed provider.
- Treats patients and families with respect and dignity; ensures confidentiality.
- Maintains a good working relationship within the department and with other departments.
- Maintains stock supply levels on the unit; notifies charge nurse or director of nursing when supplies are low.
- Orders medication from pharmacy ensuring OTC medications are not ordered and PRN medication supply is adequate.
- Documentation meets current standards and policies.
- Identifies and addresses psychosocial needs of patients and family and communicates with Social Services/Discharge Planner regarding in‑hospital and post‑discharge needs.
- Coordinates and supervises patient care under RN supervision.
- Performs aseptic procedures during treatments and special procedures.
- Responds to patient in emergency or physically distressing situations.
- Submits skill checklists and competencies on time.
- Follows United Medical Hospitals Corporate Compliance Code of Conduct and attends annual review programs and in‑services.
- Performs other duties within LPN scope of practice as assigned.
- Current Louisiana LPN Licensure.
- Current BCLS certification.
- Prior experience in rehabilitation highly desirable but not mandatory.
- OSHA:
Position has routine exposure to blood borne pathogens during normal job duties.
