Overview

As an RN Case Manager - Hospice at UnityPoint at Home, you’ll provide the right plan of care to support comfort and improve the quality of life for your patients. Working one‑on‑one with patients and their families, you’re a key part of the clinical team that surrounds the patient with support and expertise in end‑of‑life care. With a flexible schedule and independence, you can make it possible for patients to receive the important care they need where they’re most comfortable – in their homes.

The RN Case Manager - Hospice uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The RN Case Manager - Hospice develops and oversees the hospice plan of care under the direction of the physician and ensures appropriate interdisciplinary involvement to ensure the best outcome for each patient.

The RN Case Manager - Hospice delivers patient care directed by the physician as established in the hospice plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.

Responsibilities
  • Ensures compassionate, empathetic quality and safe delivery of services to patients according to care plan.
  • Consistently utilizes a patient‑centered approach considering physical, psychological/socio‑spiritual, educational, safety and related criteria appropriate to the age of the patients served in hospice.
  • Integrates and coordinates the care of the at‑risk population across the healthcare continuum making assessments, planning interventions working closely with providers and overall patient care team.
  • Facilitates the interdisciplinary plan of care to meet multiple service needs as well as promote continuity through elimination of fragmentation of care/service.
  • Delivers quality care consistent with Hospice Conditions of Participation, qualifying hospice criteria, payer coverage criteria, and providing visits/treatments consistent with the plan of care to honor patient/family values/ wishes.
Qualifications
  • Graduate of a State Board approved program for Registered Nurses.
  • Valid license as an RN in state(s) where providing care.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter:
    Child & Dependent Adult Abuse.
  • CPR:
    Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with re‑certification.
  • One year nursing experience caring for similar patient population.
  • Use of usual and customary equipment used to perform essential functions of the position.

Compensation

$10,000.00 New Hire Sign‑on Bonus. Shift: Monday‑Friday 8:00 AM – 4:30 PM, with on‑call responsibilities.
