SIGN ON BONUS: $5,000 (eligible external new hires)

SHIFT: Monday-Thursday 7:30am-4:30pm, Friday 7:30am-11:30am

Provide safe, compassionate patient care as a Licensed Practical Nurse under the direction of a healthcare provider and Registered Nurse. In this role, you will support the nursing process by monitoring patients,observing and reporting changes, and contributing to individualized care plans. You will perform nursing duties within your LPN scope of practice while promoting patient safety and comfort. • Support providers and clinical teams by preparing patients for exams, treatments, and minor procedures, including obtaining vital signs andassistingwith in-clinic workflows.
• Perform clinical tasks within scope such as administering medications and vaccines, collecting lab specimens, drawing blood, placing urinary catheters, and supporting specialty services (e.g., EKGs and cardiac testing support in cardiology settings).
• Deliver patient education using evidence-based methods and ensure high-quality,timelycare.
• Coordinate referrals, lab orders, and follow-up care to support continuity of care.
• Maintainaccurateelectronic health record documentation, complete required forms with appropriate coding, and manage tracking systems for ongoing patient monitoring and test follow-up.
• Stay engaged in clinic operations through meetings, communications, and organizational updates.
• Maintain compliance with OSHA, CLIA, HIPAA, and organizational policies whiledemonstratingprofessionalism, teamwork, reliable attendance, and a strong commitment to patient experience and quality improvement.

Education:

• Graduate of an accredited LPN program required.

Experience:

• One (1) year ofpreviousclinical experience in a medical office preferred.

License(s)/Certification(s):

• Valid LPN license in the state(s) of practice, including eligibility under an NLC multistate license required.
• Iowa Board of Nursing (IABON)
• Illinois Department of Financial and Professional Regulation (IDFPR)
• Wisconsin Department of Safety and Professional Services (WIDSPS)
• Dual licensed positions:
  Team members are required toobtain andsubmitproof of dual licensure within
  90 daysof hire/transfer. Proof of application is acceptable during this period.
• Certified Basic Life Support (BCLS) through the American Heart Association required.
• IA Child Abuse Mandatory Reporter Training in state(s) where providing ca rerequired.
