Responsibilities
  • Follow established standards of nursing practices and implement facility policies and procedures.
    • Interpret existing policies and procedures to other licensed staff, nursing assistants, residents, families and physicians.
    • Assume responsibility for compliance with Federal, State and local regulations within the assigned unit.
  • Obtain and provide reports during nurse transitions.
    • Obtain report from nurse being relieved and implement appropriate follow‑up action as necessary.
    • Provide report to nurse coming on duty, including sufficient information for follow‑up action if necessary.
  • Conduct resident rounds and direct care for C.N.A.s and L.P.N.s, initiating corrective action as needed.
    • Report changes in resident conditions to a physician, Supervisor, and responsible party promptly and take follow‑up action as necessary.
    • Identify resident problems and emergency situations, and initiate life‑saving measures in the absence of a physician.
  • Supervise and evaluate direct resident care, initializing changes or corrective action as necessary.
    • Prepare and assign nursing assistants' assignments and duties.
    • Explain all relevant resident care.
  • Provide clinical supervision of certified nursing assistants and direction to licensed practical nurses.
    • Assess the patient data collected by the licensed practical nurse.
  • Perform basic nursing skills competently.
    • Administer medications (including IV, enteral) and treatments per physician's order and facility policy.
    • Follow facility policy and State Law in handling all controlled drugs.
    • Complete admission, transfer, and discharge of residents according to facility policy.
    • Coordinate admission/discharge/transfer with physicians and other departments as necessary.
  • Perform and document comprehensive assessment of each resident.
    • Develop care plan based on assessments.
    • Participate in care conferences as requested.
    • Document medications administered or refused, effectiveness of PRN medications, treatments, wound care, general nursing notes, and related regulatory records.
    • Ensure C.N.A.s have completed their patient documentation.
  • Counsel and recommend disciplinary action when necessary.
    • Participate in the employee evaluation process by providing information on the clinical performance of certified nursing assistants to the Shift Supervisor.
  • Maintain emergency equipment and complete reporting requirements.
    • Check emergency equipment and sign off on the emergency cart checklist.
    • Order and receive medication delivery from pharmacy and check all drugs received.
    • Complete 24‑hour report sheet thoroughly.
    • Complete accident/incident reports as necessary.
    • Diligently observe infection control practices according to policy.
    • Obtain continuing education relating to the resident population.
    • Observe all facility safety policies and procedures.
    • Follow Resident's Rights policies at all times.
  • Demonstrate teamwork, attitude, and professionalism.
    • Work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies, and ancillary service providers.
    • Welcome new staff on the unit and act as a preceptor when assigned.
    • Welcome new residents and families in a positive and pleasant manner.
    • Maintain a clean, neat uniform and professional appearance.
