The Clinical Navigation Team consists of Nurse Navigators-Oncology and Patient Access Coordinators who are passionate advocates for patients navigating their cancer care journey. Nurse Navigators-Oncology are clinical experts with specialized oncology knowledge who provide compassionate, individualized support to patients, families, and caregivers—empowering them to overcome healthcare system barriers and make informed decisions across their entire care continuum. Patient Access Coordinators are administrative professionals dedicated to seamlessly coordinating services and delivering exceptional customer experiences.

Together, the Clinical Navigation Team collaborates with physicians, nurses, advanced practice providers, social workers, financial counselors, community health workers, and referring physicians to create a high-quality, efficient, safe, and truly exceptional patient experience. The Nurse Navigator-Oncology directly reports to the HFCI Manager-Clinical Navigation or Supervisor-Clinical Navigation.

• Serve as a compassionate guide and trusted resource for patients, families, and caregivers, providing individualized support that empowers them to navigate their cancer care journey with confidence and clarity
• Identify and help overcome healthcare system barriers, ensuring patients receive timely access to treatments, services, and resources they need
• Collaborate seamlessly with the interdisciplinary healthcare team to develop and implement personalized care plans that prioritize patient goals, quality of life, and evidence-based treatment approaches
• Demonstrate deep clinical expertise in oncology pathophysiology, multimodality treatment planning, symptom management, and goals-of-care conversations
• Coach and mentor patients and team members, fostering a culture of continuous learning and professional excellence
• Stay current with evolving medical and technological advances in oncology, insurance regulations, and community resources to provide the most informed guidance
• Build and maintain strong collaborative relationships both internally and externally, serving as a bridge between patients and the healthcare system
• Prioritize and reprioritize tasks with flexibility and grace, managing multiple patient needs while maintaining exceptional attention to detail
• Coordinate team efforts and support colleagues in delivering outstanding patient-centered care.

Graduate from an accredited school of nursing required. Bachelor Degree preferred.

Two (2) years of nursing experience required. Oncology experience preferred.

Outpatient/ambulatory experience preferred.

Maintains and demonstrates current clinical knowledge in area of specialty.
