Position:
Full‑Time Registered Nurse (RN) – Home Health
- Hours:
Monday–Friday, standard business hours with occasional on‑call rotation. - Work Setting:
On the road for patient visits, training at local office or remotely. - Annual salary: $70,000 – $80,000 per year (overtime eligible).
- Location:
Grove City, OH and surrounding areas (including Hillard, OH). - Travel:
Required, mileage reimbursed.
The RN plans, organizes and directs home‑care services, emphasizing community health education.
- Completes initial assessment of patients and families to determine home‑care needs.
- Provides comprehensive physical assessment, current and previous history.
- Regularly re‑evaluates nursing needs and revises plan of care as status changes.
- Uses assessment data to establish nursing diagnosis and develop a goal‑based care plan.
- Initiates preventive/rehabilitative nursing procedures and administers medications and treatments per physician orders.
- Provides direct patient care per State Nurse Practice Act, counsels patient and family, and delivers appropriate health‑care instructions.
- Identifies discharge‑planning needs and implements them prior to discharge.
- Acts as case manager when assigned, coordinating care for the caseload.
- Prepares clinical notes and updates primary physician at least every 60 days.
- Communicates with physicians regarding patient needs and changes; obtains/receives orders as required.
- Coordinates care plan with community health professionals.
- Participates in on‑call duties per policy.
- Ensures arrangements for equipment and services are available.
- Instructs, supervises, and evaluates home health aide care every two weeks.
- Graduate of an accredited nursing program (associate or bachelor’s degree).
- Valid, active nursing license.
- Licensed driver with a reliable, insured automobile.
- Valid CPR certification.
- Recent TB test.
- Prior home‑health or acute‑care experience preferred.
- Management experience preferred but not required.
- Excellent observation, verbal and written communication, problem‑solving, and basic math skills; nursing skills per competency checklist.
- Physical ability to walk, stand, reach, stoop, bend, kneel or crouch; lift and transfer patients and equipment.
- Adequate visual acuity and hearing to perform nursing skills.
