Social Worker for Mount Carmel Grove City

This role involves intervening with patients who have complex psychosocial needs, assisting with eligibility determination for social programs and funding sources, providing crisis intervention to patients and families, coordinating and facilitating the development of discharge plans for high‑risk patient populations, and receiving referrals from the multidisciplinary team. The position requires knowledge of growth and development principles across life cycles and may involve serving specific age groups.

Employment type:

Full time. Shift: Day shift Monday‑Friday with occasional weekend rotation.

Minimum Qualifications
  • Bachelor’s degree in social work
  • Current LSW license to practice in Ohio
  • Minimum 1 year experience in discharge planning in an acute or ambulatory setting
  • Effective communication skills
  • Strong organizational skills; self‑directed, flexible, and able to adapt to changing regulatory requirements
  • Excellent clinical assessment and analytical skills
  • Effective interpersonal skills; able to work across departmental boundaries, facilitate problem resolution, and maintain a professional demeanor in difficult situations
  • Knowledge of utilization management, discharge planning, care coordination, and transitions of care
  • Understanding of pre‑acute and post‑acute venues of care and community resources
  • Ability to work independently and exercise sound judgment in interactions with physicians, payers, colleagues, patients, and families
