Cities Homeless Coalition is seeking a Case Manager - Jail to Community Post-Release to support individuals experiencing homelessness in Grover Beach. You will create housing stability plans, secure permanent housing, and connect clients with community resources.
The role involves coordinating with partner agencies, referrals to CalAIM services, and maintaining client files to meet grant requirements. Evening/weekend hours may be required.
