Role Overview:

Key Responsibilities:Provide personalized support by understanding patients' medical needs, insurance, health literacy, and socioeconomic factors to overcome barriers to wellness. Assist patients in navigating healthcare systems and promote health management.
Qualifications & Skills:Requires an Associates Degree in Nursing, 2+ years of nursing experience, current RN licensure, and BLS certification. Preferred candidates have a Bachelor's Degree in Nursing, at least 1 year of case management experience, and national certification in case management.
