Yale New Haven Health - - - Responsibilities:
Read and follow the client's individual care plan and provide outlined services.;
Provide personal care to include bathing, dressing, toileting, grooming, skin care, mouth care, and shaving (with electric razor only).;
Plan and prepare meals and snacks according to dietary guidelines and encourage the client to eat a healthy diet.;
Accompany the client to medical appointments and make sure all follow-up tests or appointments are scheduled and completed.;
Maintain a safe and comfortable home environment that eliminates accident hazards.
