Nurse Practitioner

Advanced Practice Clinician - Nurse Practitioner / Physician Assistant

Program: ISNP

Schedule: Full-time, 40 hours/week

Shift: Monday-Friday, 8 AM-5 PM

Dress Code: Medical attire

Travel: Local travel within New Haven and Middlesex counties (3 facilities)

Join Optum's Senior Community Care (SCC) program, providing integrated, longitudinal care to high-risk members in facility-based settings. This position offers flexible scheduling with no on-call, weekends, or holidays.

• Perform comprehensive assessments
• Manage acute and chronic medical/behavioral conditions
• Document accurately, including ICD-10 coding
• Collaborate with an interdisciplinary care team
• Lead advanced illness and care-planning discussions
• Educate patients and caregivers

• Active, unrestricted state license (or ability to obtain)
• NP: Accredited MSN or DNP
• PA: Accredited PA program
• National board certification (AANP, ANCC, or NCCPA)
• Active DEA license
• Ability to meet physical job requirements (lifting 30 lbs, stairs, mobility)
• Reliable transportation
• Ability to obtain collaborative practice agreement (if required)
• Full COVID-19 vaccination
• Must pass pre-employment drug screening

• 1+ year clinical experience (long-term care preferred)
• Experience with complex medical, behavioral, or social needs
• Knowledge of geriatrics, chronic illness, and end-of-life care
• Proficiency with electronic medical documentation
