Behavioral Health Supervisor

Responsibilities- Ensures that each RN and LPN is effective in the assessment and care of their assigned patients according to the plan of care.
- Assigns patients to RNs and LPNs.
- Is available to receive calls from RNs and LPNs as needed.
- Ensures case conferences occur and are documented.
- Conducts supervisory home visits as necessary.
- Assists with orientation of new staff.
- Ensures that complete documentation is entered into the EMR within 24 hours of home visits.
- Works collaboratively with all staff.
- Has sufficient understanding of State regulations and Conditions of Participation. Follows policies and procedures.
- Participates in staff meetings, in-service programs, other meetings, and serves on committees as requested.
- Case management of a modified caseload as needed, per manager’s discretion.
- Performs other duties as assigned.
EDUCATION
Nursing diploma or Associate's degree in Nursing required. Bachelor's in Nursing preferred.
EXPERIENCE
Minimum of three (3) years of nursing experience preferred. Previous home care/hospice experience preferred.
LICENSURE
Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
SPECIAL SKILLS
Understanding of Medicare certification and OASIS documentation experience. Strong organizational skills and the ability to work independently. Strong computer skills and the ability to learn new software. Ability to manage a variety of assignments while establishing priorities and ensuring that necessary duties are completed.
PHYSICAL DEMAND
Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs. Requires exposure to blood and body fluids, hazardous materials, and communicable diseases.
Additional Information- 50% supervisory responsibility with the behavioral health teams in New Haven and Guilford, and 50% case management.
- Supervision of small group of staff and case management/home visits of small caseload.
- Leadership experience is desirable. Home care experience, especially in behavioral health, is desirable.
- Must have reliable transportation and valid driver's license. Must be able to work independently and as part of a team. Must be able to make decisions and problem solve.
- Monday through Friday 12pm-8:30pm
