Full time | Shift hours: 7am to 7pm

• Conduct regular direct care rounds on the assigned unit to monitor activity and ensure residents' needs are met, including assisting with activities of daily living.
• Maintain positive relationships with residents and patients.
• Notify the Charge Nurse of any resident/patient change in condition.
• Obtain vital signs, weights, and heights as instructed by the Charge Nurse.
• Complete all assigned duties and documentation as required.
• Coordinate and prepare for new admissions, transfers, and discharge of residents in the Transitional Care Unit.
• Maintain a safe and clean work area.
• Ensure infection prevention and control practices are followed.
• Other duties as assigned.

• Certified Nursing Assistant (CNA) Certification in good standing is required.
