Laurel, MS — Full Time Professional

Overview

The Care Manager supports clinical workflows that integrate value-based performance criteria. The position collaborates with patients, families, providers, and interdisciplinary teams to coordinate care for medically complex patients. The Care Manager promotes quality outcomes, safe clinical care, appropriate resource utilization, and an enhanced patient and family healthcare experience.

Minimum Qualifications
  • Current RN license by the Mississippi Board of Nursing
  • Minimum of three years of acute care clinical experience
  • Ability to read, write, and understand verbal and written communication
Preferred Qualifications
  • CPR or ACLS certification
  • Experience in care management, utilization review, population health, or healthcare revenue cycle
  • Experience with data collection and analysis
Essential Duties & Responsibilities
  • Coordinate quality care management for medically complex patients
  • Collaborate with attending physicians to establish DRGs and expected lengths of stay
  • Identify appropriate clinical resource utilization and coordinate patient referrals
  • Assess discharge needs and coordinate patient-centered discharge plans
  • Assess readmission risk and implement plans to reduce unplanned hospital readmissions
  • Support clinical workflows for compliance with value-based performance metrics
  • Maintain proficiency in evidence-based criteria guidelines, including Inter Qual
  • Educate providers regarding utilization and level-of-care decisions
  • Ensure secure and confidential access to protected health information
  • Participate in department improvement activities and interdisciplinary collaboration
Physical Requirements
  • Ability to stand and/or walk for extended periods
  • Frequent bending, reaching, squatting, and kneeling
  • Lift, carry, push, and pull up to 50 lbs independently; greater than 50 lbs with assistance/devices
  • Manual dexterity for clinical work and computer use; adequate vision, hearing, and speech
