This full‑time position supports patients in Gurnee and surrounding areas and is eligible for $20,000 total bonus potential, including a $10,000 first‑year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign‑on bonus paid within the first 18 months of employment. To receive these bonuses, you must be in good standing & meet performance expectations, and be in the role and status for which you were hired!

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self‑manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
