Position: Patient Care Associate - High Risk Antepartum - PT Evenings with Benefits (3:15pm-11:45pm)

The Patient Care Associate functions as a caregiver for a specific patient population to achieve desired outcomes. Under the direction and supervision of the registered nurse, the role promotes patient satisfaction, comfort, and safety, and assists with a variety of patient, unit, and environment-related tasks. This role also serves as Unit Services Representative, acting as a welcome ambassador to facilitate positive guest relations, perform clerical support for the unit and Nurse Manager, and participate in patient safety initiatives.

Responsibilities
  • Provide patient care based on physical, psychosocial, education, safety, and related criteria appropriate to the age of patients served in the assigned service.
  • Achieve effective and safe patient outcomes through practice, judgment, and critical actions.
  • Communicate with patients, Nurses, and other health care team members regarding patient status and care rendered.
  • Take initiative to ensure patient satisfaction.
  • Participate in interdisciplinary collaboration of the patient plan of care from admission to discharge to ensure safe transitioning and continuum of care.
  • Engage in Unit and Departmental Quality and Safety initiatives and activities.
  • Welcome patients and verify identification bracelets; prepare patients for assessment/admission, physical examination, and procedures.
  • Measure and document vital signs under RN direction and report abnormal results immediately.
  • Perform and document glucose finger sticks/heel sticks and collect other specimens as directed by the RN.
  • Observe and document patient intake and output; provide hygiene and comfort care, assist with feeding, and maintain patient safety by promptly responding to call lights.
  • Assist with patient transport, act as a patient sitter when needed, and help with non-clinical questions for patients and families.
  • Complete clerical tasks, including Advance Directives documentation and coordinating with The Office of Patient Experience.
  • Educate families on unit operations and expectations (television, telephone, meal ordering, visiting policy, parking, discharge options, etc.).
  • Act as a service recovery champion by routing inquiries/issues to appropriate personnel and proactively addressing issues.
  • Maintain a safe environment, report safety concerns, and initiate IT/equipment help desk tickets as needed.
  • Monitor inventory, order supplies, and procure items through established channels.
  • Perform A/D/T functions and patient charge entries in EPIC, including bed assignments and billing for unit-specific procedures; maintain patient medical records per policy.
  • Identify patient population needs and assist the professional nurse to tailor care to age, culture, language, and sensory needs.
  • Communicate with patients and primary caregivers at their level of understanding and adhere to organizational standards and behaviors.
Qualifications
  • High School diploma or GED; completion of one of the following programs or equivalent patient-facing healthcare experience:
    Certified Nursing Assistant (CNA), Medical Assistant (CMA), Patient Care Technician (PCT), Fundamentals of Nursing program from an accredited school of nursing, HMH PCT Apprenticeship Program.
  • Preferred: experience checking EKG, vital signs, and blood sugar.
  • AHA Basic Life Support (BLS) Certification required within 60 days of entering position.
