The Physical Therapist is responsible for, but not limited to:

• Provide compassionate, patient-centered care in the home, creating a positive and supportive experience during each visit
• Conduct in‑home assessments to evaluate patients’ mobility, functional abilities, pain levels, and overall safety
• Develop individualized treatment plans aimed at improving ADLs, strength, balance, and overall independence
• Deliver hands‑on therapy and interventions in the patient’s home using evidence‑based techniques and appropriate assistive devices
• Educate patients, families, and caregivers on exercises, mobility techniques, and safe practices to support progress between visits
• Track and evaluate patient outcomes, adjusting plans of care as needed to ensure the most effective results
• Collaborate with interdisciplinary team members to coordinate care and support smooth transitions between levels of service
• Document all visits, treatments, and progress in compliance with agency standards and home care regulations
• Maintain reliable and timely attendance to ensure continuity of care and a strong patient‑provider relationship

Master's Degree in Physical Therapy

• Current Physical Therapist Certification in the state of Operations
• 1+ years experience in Home Health, Acute Care setting or Rehabilitation Facility
