Monogram Health is seeking compassionate Advanced Practice Providers (NP/PA) to deliver high-quality in-home care to patients with multiple chronic conditions. You will work within an integrated multidisciplinary team and support a value-based care model that emphasizes population health and continuity of care in patients’ homes and via telehealth.
The role includes 3–4 home visits per week with some telehealth, no on-call requirements, and 30–90 minute visit lengths.
