Hackensack Meridian Health is seeking a Care Coordinator, Care Management to coordinate and facilitate the clinical progression of patient treatment and discharge planning. You will work with patients, families, and a multidisciplinary team to meet goals and arrange the next level of care.
Qualifications include a BSN (or progress), or MSW, with strong decision-making, communication, and computer skills. Licensure in NJ as RN or Social Worker is required; CM certifications preferred.
