FTE/Classification: Full-Time / Exempt

Location: 125 Atlantic St, Hackensack, NJ 07601

Schedule: Monday
- Friday (8:30am - 4:30pm; onsite)

Salary Range: $65,000 - $75,000 (depending on years of experience and related experience.

The Program Coordinator of Partial Care is a licensed clinician who assists the Director in general oversight and supervision of all aspects of the program, including clinical and administrative tasks. The Program Coordinator provides coverage for the Clinical Director when they are not present and coordinates the group counseling component of the program. Group Counselors report to the Program Coordinator. The Program Coordinator carries a reduced caseload and may conduct the functions of a counselor and senior counselor including caseload responsibilities, intake and member assignments and oversight of interns.

SUPERVISORY RESPONSIBILITIES:
  • Provides direct supervision to assigned employees to enhance their job skill set.
  • Provides on-going support to employees to assure completion of job responsibilities
  • Carries out supervisory responsibilities in accordance with the agency's policies, applicable laws and governmental regulatory agencies
  • Reviews employees' responsibilities to determine time frame and scope of authority, reviews employees' requirements and allotments of available resources
  • Evaluates employees' productivity and documentation of service delivery
  • Responsibilities include interviewing and training employees; planning, assigning and directing work, appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
  • PERFORMANCE RESPONSIBILITIES:
  • Works towards the accomplishment of Department/Agency goals
  • Maintains confidentiality and immediately reports to management breaches of confidentiality
  • Practices job requirements on an accurate, timely and regular basis
  • Demonstrates appropriate professional boundaries with members, co-workers, and supervisors.
  • Meets relevant continuing educational/training requirements
  • Completes all required documentation/paperwork accurately and within established time frames.
  • Rehabilitation Counselor Responsibilities:

    As a member of the counseling team the Program Coordinator may carry a reduced caseload and conduct all the functions of a counselor including:

  • Collaborates with individual clients to develop a Behavior Health Care Plan (BHCP) with treatment goals and strategies that are consistent with their current diagnosis, symptoms, and discharge planning criteria.
  • Ensures that discharge planning begins at intake.
  • Develops an individualized program schedule that includes daily groups and individual counseling. Referrals to groups are consistent with individualized treatment planning.
  • Completes the Biopsychosocial assessment at the appropriate intervals per regulation and NJFH policy.
  • Completes all Green space assessments per NJFH policy.
  • Consider pre-vocational training groups that are consistent with discharge planning goals and employment potential.
  • Work collaboratively with the members, other counselors, and rehabilitation aids to determine appropriateness, skill level, and treatment applicability for pre-vocational placement.
  • Maintain ongoing communication with outside agencies, members of the person's circle of support and medical professionals involved in member's care; documenting all contact and attempts to reach out, to ensure consistent direction of care.
  • Explore family dynamics and include family and other members of the person's circle of support in the treatment and discharge planning process.
