ResponsibilitiesThe Registered Nurse functions as a caregiver for a specific patient population in order to achieve desired outcomes. This position focuses on expanding knowledge and skills, provides effective direct care as part of the interdisciplinary team to a variety of complex patients. This position directs and oversees the care provided by other caregivers through delegation, validation, evaluation, and coordination. This position effectively coordinates patient care/unit activities among nursing peers, physicians and support services.
- Delivery of nursing practices according to evidence-based research, nursing policies, and standards of care to provide safe and individualized patient care
- Assessment, planning, implementation and evaluation: collects data pertinent to the patient’s health or situation
- Perform age and disease specific assessment/data collection independently with attention to physiologic, psychological, and cognitive status
- Recognize clinical and diagnostic status changes
- Identify education needs based on age, disease, and culturally specific differences
- Develop and coordinate a plan of care that is patient/family focused and encompasses the continuum of care
- Communicate and implement the plan of care; provide ongoing communication with patient/family, nursing colleagues, and health care team
- Coordinate and document care and interventions; provide education as needed; follow established protocols
- Provide or coordinate patient/family education and ensure understanding
- Utilize continuing point-of-care documentation; document assessments, plan-of-care, interventions and evaluation
- Medication administration using modern bar-scanning and documentation technologies
- Multidisciplinary discharging of patients for safe transition and continuity of care
- Evaluate progress toward outcomes and apply critical thinking to achieve desired results
- Identify ethical issues and address with the team; seek opportunities to enhance patient satisfaction
- Maintain competencies, certifications, and continuing education as required
- Adhere to American Nurses Association Code for Nurses with Interpretive Statements
- Lifts a minimum of 35 lbs.; pushes/pulls a minimum of 10 lbs.; stands at least 6 hours per day
- All other duties and/or projects as assigned
- Minimum of an associate degree in nursing or diploma of nursing
- BSN in progress and/or willing to acquire within three years of hire
- Current and valid Registered Nurse license for the assigned HMH location by date of hire
- BLS/CPR certification by American Heart Association within 60 days of hire
- Six months of relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program
- Ability to learn quickly, adapt to changing patient needs, strong accountability, teamwork, ongoing education, and commitment to high quality patient care
- PACU experience preferred; ED, Telemetry, and Critical Care experience preferred
- National Certification in area of specialty preferred when eligible
