Overview
Promotes individual(s) health goals by completing the nursing process for assessment. Collaborates with physicians and IDT. Identifies individual care needs, documents care services in individuals EHR. Maintains safety, asepsis, and ensures the use of universal precautions are followed . Provides quality care by adhering to therapeutic standards and measures health outcomes against individual care goals. Is available in person and remotely for triage and provides clinical support to unlicensed personnel as needed.
Ensure continuity in care. Must hold an unencumbered license for NJ State issued by the New Jersey Board of Nursing as a registered nurse. Background in home health, developmental disabilities or mental health preferred.

- Conduct comprehensive nursing assessments, including health history review, medication profile review, and risk identification (e.g., aspiration, seizure, fall, skin integrity, diabetes management).
- Monitor changes in health status, identify early warning signs, and initiate appropriate clinical interventions, referrals, and follow-up.
- Support preventive care and chronic disease management, including immunizations, routine screenings, and health maintenance planning
- participate in and provide clinical input to person-centered service planning/ISP/POC development, ensuring health and medical needs are addressed.
- coordinate care with primary care providers, specialists, behavioral health, therapists, hospitals, and community resources to ensure continuity of care.
- support transitions of care (e.g., hospital/ED discharge follow-up), including reconciliation of orders and communication of updated care needs to the team.
- Review medication regimens for appropriateness, potential interactions, side effects, and adherence challenges; elevate concerns to prescribing providers
- Provide guidance on medication administration processes (within scope and per program policy), including safe storage, documentation, and observation for adverse effects.
- Monitor and provide oversight of health-related treatments and protocols (e.g., wound care, enteral feeding, oxygen, glucose monitoring) as permitted by policy and licensure.
- Identify and document health and safety risks; recommend mitigation strategies and supports to reduce preventable incidents
- Provide consultation on infection prevention practices and outbreak response per policy.
- Ensure appropriate reporting and follow-up for incidents, sentinel events, and mandated reports in accordance with program requirements.
- Provide health education and coaching to individuals, families/guardians, and direct support staff on:
- Chronic disease self-management strategies
- Recognizing signs/symptoms requiring medical attention
- Medication side effects and adherence supports
- Nutrition/hydration, skin integrity, and general wellness
- Assist in developing and delivering training related to health plans and nursing-related supports as required
- Maintain timely, accurate clinical documentation in designated systems consistent with nursing standards and program requirements.
- Ensure confidentiality and compliance with HIPAA and applicable state and agency policies.
- Contribute to audits, quality improvement activities, and regulatory reviews as needed.
- Ability to lift, transfer, or assist individuals with mobility.
- Flexible schedule including evenings, weekends, and holidays as needed.
- Work may take place in residential homes, day programs, or community settings.
