We have an exciting opportunity for a Hospice Registered Nurse (RN) Case Manager to join our team!
Position OverviewAs an RN Case Manager, you will play a vital role in delivering exceptional hospice care by providing comprehensive nursing assessments, implementing individualized care plans, and supporting patients and their families with expert guidance and compassionate care.
Schedule: Full-time
Salary Range: $87,000 – $95,000 per year, commensurate with experience
Full-time, exempt RN Case Manager position
Mileage reimbursement is provided separately in accordance with company policy
What You’ll Do- Conduct initial and ongoing comprehensive nursing assessments to identify patient needs and develop personalized care plans
- Implement, evaluate, and adjust care plans in collaboration with the Hospice Interdisciplinary Team (IDT)
- Provide education, supervision, and emotional support to patients, families, and caregivers
- Ensure high-quality, compassionate care in accordance with hospice guidelines, regulations, and best practices
- Active RN license in New Jersey
- Minimum of two years of nursing experience, preferably in home health or hospice
- Valid driver’s license, auto insurance, and reliable transportation for field visits
- Ability to pass background checks, drug screening, and motor vehicle record review

Physical RequirementsThis role requires a combination of sitting, standing, walking, bending, reaching, and occasional lifting/carrying. Driving, documentation, and patient interaction are essential components of the position. Contact Human Resources for a full job description.
