Position Overview

We are seeking a skilled Advanced Nurse Practitioner to join our team. We partner with Holy Name Medical Center and continue to grow our footprint in Northern New Jersey.

Responsibilities
  • Manage both acute and chronic conditions
    , with a focus on continuity of care across rehab, long‑term care, and outpatient settings.
  • Provide primary care services to patients across the lifespan in a variety of settings, including outpatient offices, rehabilitation centers, and nursing homes.
  • Perform comprehensive physical examinations and obtain detailed medical histories
    .
  • Order and interpret diagnostic tests
    , including laboratory work and medical imaging, to support accurate clinical decisions.
  • Prescribe and manage medications and treatments within the NP scope of practice.
  • Conduct health coaching and patient education
    , promoting wellness, preventive care, and chronic disease management.
  • Collaborate closely with physicians, therapists, nursing staff, and other healthcare professionals to develop and implement coordinated patient care plans.
  • Provide specialized follow‑up care in rehab and nursing home populations, addressing issues such as mobility, pain management, medication compliance, and discharge planning.
  • Utilize electronic health record systems (e.g., eClinical

    Works) to accurately document encounters, coordinate care, and track patient progress.
  • Maintain high standards of infection control and aseptic technique in all procedures and care settings.
  • Support transitions of care by communicating effectively between rehab/nursing facilities and outpatient office staff.
Qualifications
  • Valid Nurse Practitioner license in the state of NJ.
  • Master's degree in Nursing with a specialization as a Nurse Practitioner.
  • Certification as a Nurse Practitioner in the appropriate specialty area (e.g., Family, Pediatrics, Adult‑Gerontology).
  • Experience in primary care or a related field preferred.
  • Strong clinical skills and knowledge of evidence‑based practice guidelines.
  • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
