Under the direction of local leadership, the Oncology Nurse Navigator (ONN) navigates the patient's journey starting with pre‑diagnosis and continuing throughout the treatment continuum. In collaboration with the multidisciplinary team—including primary care physicians, surgeons, gynecologists, gastroenterologists, oncologists, and the entire health care team—the ONN supports oncology patients and facilitates access to timely, comprehensive care. The ONN makes referrals as appropriate to services such as financial counseling, nutrition, social work, and others.
Responsibilities- Coordinates and navigates outpatient care for oncology patients, identifying new patients during the pre‑diagnosis journey and offering services from diagnosis to survivorship or end of life.
- Builds and maintains professional relationships with oncologists, radiation oncologists, surgeons, and other medical professionals involved in patient care.
- Establishes communication with physicians, physician extenders, and office staff to outline navigation programs and available resources; collaborates with the primary medical oncologist or radiation oncologist and/or oncology surgeon as a patient and family resource from initial consultation through treatment.
- Maintains a communication system that ensures comprehensive, timely information flow to physicians, nursing staff, and the interdisciplinary team.
- Facilitates access to services, tracks patient flow through diagnostic and treatment services, documents assessments and follow‑up notes in the medical record via SMART PHRASE, and provides monthly activity reports to the Director/RN Manager/designee.
- Provides ongoing assessment of patient needs—including educational, financial, psychosocial, nutritional, and clinical—making referrals as needed and connecting patients and families to resources within the health system and the community.
- Works collaboratively with the research department.
- Participates and prepares for Tumor Board meetings as needed.
- Develops survivorship care plans for applicable patients per Commission on Cancer requirements, distributing and discussing plans with patients, families, and other providers.
- Participates in performance improvement activities and guideline development with medical directors and physician leaders to improve outcomes, assure resource utilization, and increase satisfaction.
- Participates in department‑specific committees, represents patient navigation services, and reports on patients served, ensuring all regulatory standards for patient care are met.
- Represents the Cancer Program’s services to the community, collaborating with marketing for brochures and materials, identifying community partners, and promoting service awareness.
- Performs other duties and projects as assigned.
- Adheres to HMH Organizational competencies and standards of behavior.
- Bachelor of Science in Nursing (BSN) – applicable to team members hired on or after March 2024.
- NJ State Professional Registered Nurse License.
- Minimum of two years of nursing experience.
- Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
- Oncology experience preferred.
- Certified Oncology Nurse preferred.

Base compensation may vary based on market data, experience, education and certifications, skills, geographic location, internal equity, and budget or grant funding.
