Oncology Nurse Navigator - JTCC Third Floor

About This Role

The Oncology Nurse Navigator (ONN) at Hackensack Meridian Health guides patients through their cancer journey, from pre‑diagnosis to survivorship or end of life. This role is ideal for those who excel in collaboration and patient advocacy within a multidisciplinary team.

• Location: Hackensack, NJ
• Facility: Hackensack Meridian Health
• Schedule: Full-time, Day shift

• Coordinate and navigate outpatient care for oncology patients.
• Identify new patients during the pre‑diagnosis journey and offer navigation services.
• Build and maintain professional relationships with medical oncologists, radiation oncologists, and surgeons.
• Establish communication with physicians and their office staff regarding navigation programs.
• Collaborate with the primary medical oncologist and oncology surgeon to support patients and families.
• Maintain a communication system for timely information transmission to the healthcare team.
• Facilitate access to services and track patient flow through diagnostic and treatment services.
• Document assessments and follow‑up notes in patients' medical records.
• Provide monthly reports of activity to the Director/RN Manager/designee.
• Develop and participate in performance improvement activities to enhance patient outcomes.

Education:

• Bachelor of Science in Nursing (BSN) required

Experience:

• Minimum of 2 years of experience in nursing

Licenses &

Certifications:

• Current NJ State Professional Registered Nurse License required
• Oncology Certified Nurse preferred
