RN Case Manager, Per Diem

• Coordinate patient care progression throughout the continuum.
• Facilitate collaborative management of patient care, intervening to remove barriers.
• Apply process improvement methodologies to evaluate care outcomes.
• Communicate with physicians, nursing, and ancillary staff.
• Evaluate care against Length of Stay metrics.
• Collaborate with Admitting for accurate patient demographic and insurance information.
• Communicate medical plan of care and discharge plan to patients and families.
• Assess and plan for discharge needs and arrangements.
• Complete utilization management for assigned patients.
• Monitor length of stay and clinical resource use.

• Bachelor's degree in nursing (BSN) preferred; 1 year experience for non-BSN HackensackUMC employees.

• 2+ years clinical experience in relevant specialty preferred.
• Minimum of 2 years in Case Management in an acute care setting preferred.

• Current NJ State Professional Registered Nurse License required.
• BLS AHA Basic Health Care Life Support HCP Certification required.
