RN Case Manger, Per Diem

The RN Case Manager, Per Diem at Hackensack Meridian Health is responsible for coordinating patient care activities to ensure compliance with evidence-based practice standards and regulatory requirements. This role is ideal for a skilled nurse who excels in collaboration and communication within a multidisciplinary team.

• Facilitate collaborative management of patient care across the continuum.
• Apply process improvement methodologies in evaluating outcomes of care.
• Coordinate communication with physicians, nursing, and ancillary staff.
• Evaluate care provided against the Length of Stay.
• Collaborate with Admitting to ensure accuracy of patient demographic and insurance information.
• Communicate with patients and families regarding medical and discharge plans.
• Assess and plan for discharge needs and arrangements.
• Complete utilization management for assigned patients.
• Monitor length of stay and clinical resource use on an ongoing basis.
• Manage all aspects of discharge planning for assigned patients.

• Bachelor's degree in nursing (BSN) preferred; 1 year experience for non-BSN HackensackUMC employees.

• At least 2 years clinical experience in the relevant clinical specialty preferred.

• Current NJ State Professional Registered Nurse License required.
• BLS AHA Basic Health Care Life Support HCP Certification required.
