The Nurse Navigator - Oncology at Hackensack Meridian Health guides patients through their cancer journey, from pre-diagnosis to survivorship or end of life. This role is ideal for those who excel in collaboration and patient advocacy within a multidisciplinary team.

  Part-time, Day shift

• Coordinate and navigate outpatient care for oncology patients.
• Identify new patients during the pre-diagnosis journey and offer navigation services.
• Build and maintain professional relationships with medical oncologists, radiation oncologists, and surgeons.
• Establish communication with physicians and their office staff regarding navigation programs.
• Collaborate with the primary medical oncologist and oncology surgeon to support patients and families.
• Maintain a communication system for timely information transmission to the healthcare team.
• Facilitate access to services and track patient flow through diagnostic and treatment services.
• Document assessments and follow-up notes in patients' medical records.
• Provide monthly reports of activity to the Director/RN Manager/designee.
• Develop and participate in performance improvement activities to enhance patient outcomes.

Education:

• Bachelor of Science in Nursing (BSN) required

Experience:

• Minimum of two years of experience in nursing

Licenses &

Certifications:

• Current NJ State Professional Registered Nurse License
• Oncology Certified Nurse preferred
