Capacity Management RN - Patient Placement

About This Role

The Capacity Management RN is responsible for coordinating patient placement, access case management, and patient flow to ensure effective and efficient patient care. This role is ideal for those with strong clinical expertise and the ability to communicate effectively across various departments.

• Location: Hackensack, NJ
• Facility: Hackensack Meridian Health
• Schedule: Full-time, Day shift

• Coordinate the placement of elective and emergent admissions and transfers from other facilities.
• Ensure patients are admitted or transferred to the most appropriate available bed.
• Communicate and implement the Capacity Management plan for the day.
• Review patient conditions for appropriate placement in conjunction with case management staff.
• Track bed availability and maintain appropriate logs/records.
• Work with Plant Ops and Environmental Services on daily Room of the Day (ROD).
• Conduct walking rounds on nursing units to identify barriers to discharge.
• Monitor bed availability and pending discharges to meet ADT needs.
• Act as a resource for patient placement staff regarding unit acceptance criteria.
• Communicate with the Director for Capacity Management regarding operational needs.

• Education: Bachelor's degree in Nursing from an accredited nursing program
• Experience: 3 years of professional nursing experience
