Always Best Care - - - Responsibilities:
Assess new clients, develop plans of care, and orient Certified Homemaker-Home Health Aides to cases.;
Conduct client reassessments every 60 days, or more frequently if the client's condition warrants.;
Assess Aide competency and observe their performance every 3 months.;
Maintain documentation per New Jersey Board of Nursing/NJ Division of Consumer Affairs regulations and ACHC accreditation standards.;
Be available for off-hour phone consults (very rare).
