Description
Under the direct supervision of the Chief Medical Officer (CMO), SVP of Medical Services, and/or Clinical Site Director, the Nurse Practitioner (NP) provides direct clinical care to patients as per education, training, license, and certification. The NP is a member of the Care Team with the purpose of providing evidence‑based and quality medical care to Zufall Health Center patients.

Essential Functions, Duties and Responsibilities- Provides care to patients in accordance with rights and privileges granted through RN licensure and NP certification including diagnostic and therapeutic assessments, treatment of conditions and illnesses within scope of practice, referral and follow‑up of patients, and other duties as assigned; serves as a primary care provider to an appropriate panel of patients and acts as patient advocate.
- Works closely with collaborating physician and follows the guidelines detailed in the joint protocol for NPs and collaborating physicians.
- Leads assigned clinical support staff within the patient care team to improve patient care.
- Participates in night and weekend call coverage in rotation with other providers as assigned. Consults with collaborating physician or CMO on cases as needed. Assists with weekend, night and early morning hours of operations and adjusts schedule as needed.
- Assists patients in obtaining community resources, referrals and other services as clinically indicated by the center’s diverse populations. Assists the CMO and collaborating physician in accessing services for the center and its patients with the local provider community, including physicians, hospitals, and laboratories.
- Remains mindful of and achieves federal and health center productivity standards.
- Participates in clinical performance improvement activities and conducts peer reviews of meetings as deemed necessary by the CMO. Attends regular meetings of the medical staff and other meetings as deemed necessary by the CMO.
- Participates in clinical operations by ensuring compliance with local, state, and federal rules and regulations, and ensures that licensing, credentialing, and educational activities are kept current and up to date. Contributes to and follows established policies and protocols for clinical care.
- Participates in in‑service educational programs and continuing education. Teaches and trains other health center staff and NP students as needed.
- Participates in community health initiatives including health fairs, special immunization sessions, and other community and outreach activities as directed.
- Maintains membership in appropriate professional associations. Is an active member of the Medical Staff at Zufall Health Center.
- Follows code of conduct, policies and procedures of Zufall Health Center, and state and federal regulations.
- Maintains client confidentiality and follows HIPAA regulations in all interactions.
- Performs other related duties as assigned by the CMO, or other supervising physician.
- Assumes duties of Director of Nursing as assigned by the SVP of Nursing and Clinical Operations.
To perform successfully, the individual must be able to perform the essential duties satisfactorily. The requirements below are representative of the knowledge, skill, and ability required.
- Excellent verbal and written communication skills.
- Able to relate well to a diverse group of patients and staff.
- Able and willing to work on an as‑needed basis to meet operational needs of the center.
- Proficient with computers, including knowledge of Microsoft Office applications (Word and Excel) and electronic medical records.
- Learn and apply trauma‑informed care principles in everyday interactions.
- Ability to speak both English and Spanish strongly preferred.
- Bachelor’s degree as a Registered Nurse and Master’s degree or Doctorate from an Advanced Nurse Practitioner training program from an accredited college or university.
- Certification as an NP in the area of training.
- Possession of an RN license and an Advanced Nurse Practitioner license from New Jersey.
- Current certification as an Advanced Nurse Practitioner in specialty practice.
- Possession of a valid DEA and CDS registration. BLS certification.
- Minimum of one year experience in a community health setting preferred.
- Bicultural and bilingual experience valued.
$110,000–$121,450 per year
