Position:
Nurse Practitioner – Palliative & Primary Care

Essential FunctionsProvide palliative and primary care services, focusing on symptom management, quality of life, and goal-concordant care
Conduct comprehensive history and physical exams for patients with complex acute, chronic, and serious illnesses
Lead and document goals-of-care discussions, including advance care planning and code status conversations
Facilitate completion and updates of advance directives, POLST/DNR orders, and end-of-life care preferences
Manage pain and non-pain symptoms (e.g., dyspnea, agitation, nausea, anxiety, depression) using pharmacologic and non-pharmacologic approaches
Review abnormal vitals and laboratory findings and implement appropriate interventions or elevate to the attending physician as needed
Evaluate and manage new admissions, ensuring alignment of treatment plans with patient goals and prognosis
Participate in readmission reviews, identifying opportunities to improve symptom control and prevent avoidable hospitalizations
Collaborate with the interdisciplinary team (IDT) weekly to review patient status, prognosis, and care plans
Identify patients appropriate for palliative or hospice referral and facilitate timely transitions of care
Support care transitions (admissions, transfers, discharges) with emphasis on continuity, comfort, and patient-centered goals
Provide education and emotional support to patients and families regarding disease trajectory, prognosis, and end-of-life expectations
Collaborate with multidisciplinary teams including nursing, social work, chaplaincy, and therapy services
Monitor effectiveness of interventions and adjust care plans to maintain comfort, dignity, and physiologic stability
Serve as a clinical resource to facility staff on palliative care principles, symptom management, and end-of-life care
Master’s (MSN) or Doctorate (DNP) Nurse Practitioner program completion
Experience in primary care, geriatrics, or post‑acute care preferred, but not required
Palliative care training and certification provided
