Position: Home Care Field Nurse (RN) -- per diem

Part-Time Registered Nurse (RN)

We are seeking a part-time Registered Nurse (RN) to join our dedicated home care team. Our private-duty agency provides personal care (non-medical) services to the elderly in Morris and Essex counties. The field nurse will perform in-home client assessments, create individualized care plans, and orient and monitor aides providing services to our clients.

Job Duties:

Assess new clients, develop plans of care, and orient Certified Homemaker-Home Health Aides to cases.

Conduct client reassessments every 60 days, or more frequently if the client's condition warrants.

Assess Aide competency and observe their performance every 3 months.

Maintain documentation per New Jersey Board of Nursing/NJ Division of Consumer Affairs regulations and ACHC accreditation standards.

Be available for off-hour phone consults (very rare).

Requirements:

Current RN license in good standing in NJ.
Valid NJ driver's license and reliable vehicle to get to clients' homes.
Minimum two years RN experience, including one year of home health care experience preferred.
Excellent clinical, assessment, organizational, and communication skills.
Ease with computers.
Some schedule flexibility.
