Job Overview

BAYADA Home Health Care is seeking a compassionate and dedicated Full Time Registered Nurse (RN) hospice case manager. The position is located in Pennsauken, NJ, servicing hospice clients on a per‑visit basis in Camden and Burlington counties.

• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, procedures, and comfort care.
• Monitor clients' conditions and report changes to the Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
• Occasional on‑call after hours and/or weekends.

• Current license as a Registered Nurse (RN) in New Jersey.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Base Pay: $43–$45 per hour, depending upon qualifications and subject to negotiation.
