Adoration Home Health and Hospice is seeking a passionate, dedicated Licensed Practical Nurse (LPN) to join our Home Health team in Hagerstown, MD. Our LPNs provide expert, patient‑centered care in a supportive, fulfilling environment where your skills and empathy truly shine.

Full‑time, Monday – Friday

• Providing 1:1 care for clients following their plan of care at all times while maintaining client dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client’s plan of care and providing assessments of results
• Accurately documenting nursing actions of all care given, communicating with family, case manager, and physicians, and documenting progress and outcomes for established goals, informing physicians, case managers, and families of changes in client’s medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
• Ensuring all patient needs are met in accordance with physician instructions

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ year of nursing experience preferred
• Home health care experience preferred

USD $28.00 - $33.00 / Hour
