Position Summary
Under limited supervision and in collaboration with the medical staff, the Nurse Practitioner provides advanced, comprehensive healthcare services to a designated patient population. This role is responsible for delivering high-quality, patient-centered care while adhering to established nursing and medical standards. The Nurse Practitioner collaborates closely with physicians, nurses, and other members of the healthcare team to ensure optimal patient outcomes.
Key Responsibilities Patient Assessment and Care- Perform comprehensive medical examinations, evaluations, diagnoses, treatment, follow-up care, consultations, and health education appropriate to the patient's age and condition.
- Obtain detailed patient histories and develop individualized plans of care while ensuring accurate and complete documentation.
- Order, perform, and interpret appropriate diagnostic and laboratory tests.
- Develop, implement, and document treatment plans, including prescribing medications based on a thorough review of patient history and diagnostic findings.
- Provide medical care to ill and injured patients in accordance with accepted standards of practice.
- Collaborate with physicians, primary care providers, referring providers, nursing staff, case managers, and other healthcare team members to ensure continuity of care.
- Communicate treatment plans and health information effectively to patients and their families.
- Provide education regarding disease processes, medications, treatment plans, and health maintenance strategies.
- Initiate consultation and coordinate with other healthcare providers as necessary to optimize patient outcomes.
- Maintain accurate and timely documentation of patient assessments, treatments, laboratory and radiographic results, and clinical findings.
- Evaluate patient outcomes and collaborate with the healthcare team to modify care plans as needed.
- Protect the privacy and confidentiality of all patient information in accordance with HIPAA and organizational policies.
- Promote patient safety and provide evidence-based care that supports quality outcomes.
- Master of Science degree with completion of an accredited Nurse Practitioner program required.
- Current Nurse Practitioner license in the state of practice required.
- Current Registered Nurse license in the state of practice required.
- Current BLS certification, or the ability to obtain certification prior to providing direct patient care, required.
- DEA and CDS eligibility required.
- Previous Nurse Practitioner experience preferred, but new graduates are encouraged to apply.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Strong organizational, critical thinking, and documentation skills.
- Commitment to providing compassionate, patient-centered care and maintaining the highest standards of professionalism.
