Position Summary

Under limited supervision and in collaboration with the medical staff, the Nurse Practitioner provides advanced, comprehensive healthcare services to a designated patient population. This role is responsible for delivering high-quality, patient-centered care while adhering to established nursing and medical standards. The Nurse Practitioner collaborates closely with physicians, nurses, and other members of the healthcare team to ensure optimal patient outcomes.

• Perform comprehensive medical examinations, evaluations, diagnoses, treatment, follow-up care, consultations, and health education appropriate to the patient's age and condition.
• Obtain detailed patient histories and develop individualized plans of care while ensuring accurate and complete documentation.
• Order, perform, and interpret appropriate diagnostic and laboratory tests.
• Develop, implement, and document treatment plans, including prescribing medications based on a thorough review of patient history and diagnostic findings.
• Provide medical care to ill and injured patients in accordance with accepted standards of practice.

• Collaborate with physicians, primary care providers, referring providers, nursing staff, case managers, and other healthcare team members to ensure continuity of care.
• Communicate treatment plans and health information effectively to patients and their families.
• Provide education regarding disease processes, medications, treatment plans, and health maintenance strategies.
• Initiate consultation and coordinate with other healthcare providers as necessary to optimize patient outcomes.

• Maintain accurate and timely documentation of patient assessments, treatments, laboratory and radiographic results, and clinical findings.
• Evaluate patient outcomes and collaborate with the healthcare team to modify care plans as needed.
• Protect the privacy and confidentiality of all patient information in accordance with HIPAA and organizational policies.
• Promote patient safety and provide evidence-based care that supports quality outcomes.

• Master of Science degree with completion of an accredited Nurse Practitioner program required.

• Current Nurse Practitioner license in the state of practice required.
• Current Registered Nurse license in the state of practice required.
• Current BLS certification, or the ability to obtain certification prior to providing direct patient care, required.
• DEA and CDS eligibility required.

• Previous Nurse Practitioner experience preferred, but new graduates are encouraged to apply.
