Job OverviewUnder the supervision of the Medical Director, the Nurse Practitioner II at Kolmac Integrated Behavioral Health and its subsidiaries is responsible for providing evidence-based, integrated treatment to patients with co-occurring mental health and substance use disorders.
Essential Duties and Responsibilities- Provide integrated behavioral health services to patients both virtually and in person.
- Assess, diagnose, and treat adult patients with co-occurring substance use and mental health disorders; establish medical diagnoses based on history, assessment, and diagnostic findings.
- Collaborate with counselors, case managers, or other treatment team members to coordinate client care.
- Complete medical records accurately and timely, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
- Examine or order laboratory and diagnostic tests for patients; interpret appropriate findings.
- Design, order, and document treatment plans, including prescriptions based on comprehensive review.
- Initiate protocols and clinical pathways; evaluate the plan of care to ensure timely intervention.
- Perform invasive procedures when needed.
- Collaborate with primary care providers regarding admission, discharge, and transfer decisions.
- Present patients in the caseload during rounds.
- Communicate plan of care with providers, patients, and families; provide instruction regarding treatment and medication regimens.
- Counsel and educate patients and families about disease processes, health maintenance, and promotion activities.
- Initiate appropriate referrals and specialty consultations as needed.
- Facilitate patient flow and collaborate with multidisciplinary team, including case management, to ensure timely discharge.
- Provide specialty consultation and advocate for patients to ensure participation and understanding of treatment options.
- Consider holistic needs and orchestrate resources such as pain service and palliative care.
- Perform other duties as assigned.
- Provide formal and informal educational programs for other health care team members.
- Participate in mentoring activities and precept interns.
- Promote understanding of disease processes in the specialty/program.
- Act as a clinical resource for interns, residents, fellows, nurses, and medical students.
- Provide community education, health promotion, and outreach.
- Serve as a community health resource.
- Coordinate adequate nurse practitioner coverage on the service.
- Work with physicians and nurse practitioner staff to ensure coverage.
- Collaborate with primary care providers to meet service-specific outcome targets.
- Participate in performance improvement activities to enhance clinical outcomes.
- Develop clinical pathways in collaboration with physician leadership.
- Lead quality assurance activities and establish standards of practice.
- Advance patient care delivery through research, evidence-based practice, and best practices.
- Advise and influence policies to improve delivery of care.
- Introduce and evaluate new care delivery systems and interventions.
- Participate in departmental and division meetings and organizational initiatives.
- Maintain compliance and regulatory documentation for administrative purposes.
- Participate in accreditation readiness.
- Supervise ancillary staff and conduct annual reviews.
- Model and uphold customer service focus for all internal and external customers.
- Demonstrate respect for all people in the work environment.
- Ensure confidentiality of patient information.
– Education and Experience Must Haves
- Must not be in recovery while working with DEA–regulated medications.
- Active or pending CDS & DEA registration.
- Understanding of DSM‑V and diagnosis codes.
- Organizational and time‑management skills.
- Strong writing and oral communication skills.
- At least 1 year of experience working with the SUD population.
$70 – $85 hourly (depending on experience).
