Job OverviewUnder the supervision of the Medical Director, the Nurse Practitioner II at Kolmac Integrated Behavioral Health and all its subsidiaries is responsible for providing evidence-based, integrated treatment to our patients. The NP II assesses, diagnoses and provides continuity care for patients with co-occurring mental health and substance use disorders.
Essential Duties and Responsibilities- Provide integrated behavioral health services to patients both virtually and in person.
- Assess, diagnose, and treat adult patients with co-occurring SUD and mental health disorders; establish medical diagnoses based on history, assessment, and diagnostic findings.
- Collaborate with counselors, case managers, or other treatment team members to coordinate client care.
- Complete medical records accurately and timely, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
- Order, obtain, and interpret appropriate laboratory or diagnostic tests for clients.
- Design, order, and document treatment plans, including prescriptions of medications based on a comprehensive review of health information and diagnostic results.
- Initiate protocols/clinical pathways and evaluate the plan of care to ensure timely intervention.
- Perform invasive procedures when needed.
- Collaborate with primary care providers regarding admission, discharge, and transfer decisions.
- Present patients in the caseload during rounds.
- Communicate the plan of care with appropriate providers and with patients and family members.
- Provide patient and family instruction related to the plan of care, disease process, new treatment plans, and medication regimens.
- Counsel and educate patients and family members in disease processes, health maintenance, and health promotion activities; provide primary, secondary, and tertiary preventive care services.
- Initiate appropriate referrals and specialty consultations as needed.
- Facilitate patient flow and collaborate with the multidisciplinary team to ensure appropriate and timely discharge.
- Provide specialty consultation.
- Advocate for patients, ensuring family participation in care, understanding of treatment options, and patient rights.
- Consider holistic needs of patients and orchestrate resources such as pain service or palliative care.
- Perform other duties as assigned.
- Provide formal and informal educational programs for other members of the health care team consistent with evidence-based practice standards.
- Participate in mentoring activities and precept interns.
- Promote understanding of disease processes in the area of specialty/program.
- Act as a clinical resource for other team members including interns, residents, fellows, nurses, and medical students.
- Provide community education, health promotion, and outreach.
- Act as a community health resource.
- Coordinate adequate nurse practitioner coverage on the service.
- Work with physician and nurse practitioner staff to ensure coverage.
- Work with primary care providers to achieve service‑specific outcome targets for key metrics, including clinical outcomes, level of service, cost of care, and patient satisfaction.
- Participate in performance improvement activities aimed at improving clinical outcomes and minimizing variation.
- Collaborate with physician leadership and the entire leadership team to develop clinical pathways.
- Participate in and lead quality assurance activities such as chart reviews and peer review; establish standard of practice.
- Advance the patient care delivery process through application of research, evidence-based practice standards, and industry best practices.
- Advise and influence CCG policies and procedures to improve the delivery of care.
- Introduce and evaluate new patient care delivery systems, models of care, and therapeutic and preventive interventions that address patient needs not met by current care strategies.
- Actively participate in departmental and division meetings and organizational initiatives.
