What You'll Do

As a Care Manager RN, you will:

* Manage a panel of high-risk, high-cost, and chronic condition patients

* Assess patient needs and develop individualized, patient-centered care plans

* Collaborate with primary care physicians and multidisciplinary teams to coordinate care

* Coach and educate patients on disease management, preventive care, medications, and self-management strategies

* Connect patients with community resources and support services

* Promote engagement in healthy behaviors and self-care activities

* Facilitate smooth transitions across the continuum of care, including acute care, rehabilitation, skilled nursing, home health, and outpatient settings

* Monitor patient progress and adjust care plans to support optimal outcomes

* Utilize evidence-based practices to improve quality, efficiency, and patient satisfaction

* Advocate for patients, families, and caregivers throughout their healthcare journey

Qualifications

Required Education

* Associate Degree in Nursing (ASN)

Preferred Education

* Bachelor of Science in Nursing (BSN)

Required Experience

* ASN with 10 years of acute care nursing experience; OR

* BSN with 5 years of acute care nursing experience

Licensure & Certifications

* Active Registered Nurse license in Maryland or qualifying Nurse Licensure Compact (NLC) multi-state license

* Current BLS/CPR certification

* Ability to obtain active Pennsylvania and West Virginia RN licenses within 6 months of employment

* Care/Case Management Certification required within 5 years of employment

Preferred Experience

* Care management, case management, managed care, patient-centered medical home, or integrated care management experience

* Experience working with chronic and complex patient populations
