Home Health RN Care Coordinator

The Home Health RN Care Coordinator would work with physicians and multidisciplinary team members to develop a plan of care for assigned patients. You will plan, implement, and document nursing care based on patient’s needs and ensure progress toward desired outcomes by monitoring care through assessment and patient records. St. Luke’s Home Health and its interdisciplinary team values integrity, compassion, accountability, respect, and excellence with all patients receiving home health.

The team works collaboratively to provide quality health care to patients and families in the community.

Reliable personal transportation is required during work hours with current auto registration and auto insurance.

Hours will be between Monday – Friday (8:30 am – 5:00 pm) with rotating/variable weekends including rotating/variable weekday and weekend daytime/evening call up to 8:00 pm.

• Nursing degree
• Experience:
  
  2 years of RN experience
• Current RN license in the state of practice
• Valid driver’s license and auto insurance if travel is required
• Current Basic Life Support (BLS) Provider certification through American Safety and Health Institute, American Heart Association, or American Red Cross
