St. Luke’s Health System in Idaho is seeking a Home Health RN Care Coordinator. You will work with physicians and the interdisciplinary team to develop and implement plans of care for assigned patients, document nursing care, and monitor progress toward outcomes. Reliable transportation required.
Hours Monday–Friday 8:30am–5:00pm with rotating weekends and on-call. Requires RN license, 2 years RN experience, BLS certification, and valid driver’s license if travel is required. St.
