What You Can ExpectThe Home Health RN Care Coordinator will work with physicians and multidisciplinary team members to develop a plan of care for assigned patients. You will also plan, implement, and document nursing care based on patient’s needs, ensuring the patient is progressing toward desired outcomes by monitoring care through assessment and/or patient records. St. Luke’s Home Health and its interdisciplinary team values integrity, compassion, accountability, respect, and excellence with all the patients receiving home health.
The team collaborates to provide quality health care to patients and families in the community.
Reliable personal transportation is required during work hours with current auto registration and auto insurance.
Hours:
Monday - Friday (8:30am - 5:00pm) with rotating/variable weekends including rotating/variable weekday and weekend daytime/evening call up to 8:00 pm.
- Nursing Degree
- Experience:
2 years of RN experience - Licenses/Certifications:
Current RN License in the State of Practice - Valid Driver's License and Auto Insurance if work related travel is required
- Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
