Job Title: Transition Support
Reports To: Coordinator of Secondary Special Education
Position SummaryThe Transition Support staff member works collaboratively with students, families, educators, and community partners to support high school students with disabilities in preparing for postsecondary education, employment, independent living, and community participation. This position provides direct instruction, career development, and employment support through individualized and small-group activities while helping students achieve their transition goals.
Student Transition Services- Provide direct transition instruction in individual and small-group settings.
- Facilitate lessons and activities focused on self-advocacy, independent living, financial literacy, career exploration, workplace readiness, communication skills, and postsecondary planning.
- Support students in developing and achieving individualized transition goals aligned with their IEPs.
- Monitor student progress and collaborate with special education staff regarding student growth.
- Prepare students for employment through instruction in job applications, resumes, interviewing, workplace expectations, and employment skills.
- Coordinate and support job shadowing, volunteer experiences, community-based instruction, and work-based learning opportunities.
- Develop relationships with local employers and community agencies to expand student employment opportunities.
- Support students during employment placements and provide coaching to promote successful work experiences.
- Coordinate and oversee the Hamden Public Schools Work Study Program.
- Recruit, place, and support student employees at district worksites.
- Conduct worksite visits and collaborate with supervisors to ensure successful student employment experiences.
- Monitor student participation and communicate regularly with students, families, school staff, and employers.
- Serve as the district liaison with community agencies, including the Bureau of Rehabilitation Services (BRS), Level Up, Junior Achievement, and other transition partners.
- Coordinate transition workshops, guest speakers, career exploration events, and community-based learning experiences.
- Connect students and families with postsecondary education, employment, and adult service resources.
- Experience working with adolescents, individuals with disabilities, or transition-age youth.
- Knowledge of transition planning, career development, and work-based learning.
- Strong communication, collaboration, and organizational skills.
- Ability to build positive relationships with students, families, school staff, employers, and community agencies.
- Experience using educational technology and career planning resources preferred.
