Community Companion Home Coordinator (CCH)
Aspire Living & Learning seeks a Community Companion Home Coordinator to lead, oversee, and provide hands‑on support to Home Care Providers, ensuring health, safety, and well‑being of individuals in shared living arrangements. The role requires extensive travel throughout Connecticut—visiting homes, meeting providers, and connecting with community residents. CCH builds trusting relationships with providers and clients, fostering personal growth, meaningful connections, and high quality of life.
The coordinator collaborates with Aspire management, state agencies, families, medical professionals, and stakeholders to create inclusive environments.
- Provide leadership, oversight, and hands‑on support to Home Care Providers.
- Travel to homes, meet with providers, and connect with residents across Connecticut.
- Build trusting relationships with providers and individuals to ensure personal growth and meaningful connections.
- Collaborate with Aspire management team, state agencies, families, medical professionals, and other stakeholders to create supportive, inclusive environments.
- Bachelor’s degree or equivalent professional experience.
- Prior experience supporting individuals with disabilities is strongly preferred.
- Reliable transportation, valid driver’s license, and willingness to travel.
- Strong communication skills, empathy, and enthusiasm for making a difference.
- Basic computer skills (experience with Therap is a plus).
Pay Rate: $58,000 per year.
Hours:
40 flexible Monday through Friday various times and weekends.
