We provide skilled nursing care to people recently discharged from the hospital, needing therapy and long-term care.

- Conduct psychosocial assessments, evaluating a resident's emotional, social, and psychological history.
- Develop and maintain individualized care plans, collaborating with the healthcare team and updating interdisciplinary team on resident progress.
- Provide counseling to help residents cope with grief, loss, depression, anxiety, and transition to nursing home.
- Advocate for resident rights, ensuring voices are heard and needs are met.
- Resolve grievances, mediate complaints, and report suspected abuse or neglect.
- Coordinate discharge plans, connecting residents with community resources like home health or hospice.
- Assist in connecting residents and families to legal, financial, insurance, and case management services.
- Guide residents and families through advanced directives and end-of-life support.
- Licensed to work within Connecticut as an LSW.
- Pass background check, drug screen, professional references check, and physical evaluation.
- Strong leadership, customer service and clinical competency.
- Leadership experience in a long-term care setting.
