Physical Therapist – Greater Hartford, CT
FOX Rehabilitation is seeking licensed Physical Therapists to provide individualized, one-on-one care to older adults in private residences and senior living communities across Hartford, Windsor, Coventry, Middletown, Glastonbury, Southington, Farmington and Wethersfield.
Responsibilities- Provide one-on-one physical therapy to older adults at home, senior living and assisted‑living settings.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatments.
- Deliver therapy sessions that support patient wellness, mobility, functional independence and longevity.
- Document all interactions accurately within FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs and fitness specialists.
- Educate patients, caregivers and senior living staff on safety, function and aging in place.
- Provide telehealth or virtual care when appropriate to extend access and flexibility.

- Full‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits).
- Part‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits, eligibility based on minimum hours).
- PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit compensation).

- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain it.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Lift 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs continuously.
- Perform CPR.
- Provide moderate to maximum assistance with patient transfers and bed mobility.
- Reposition patients; stand, walk and climb stairs for extended periods; perform therapeutic activities.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Wear PPE when required.
