Pediatric Care Navigator: Access & Care Coordination

Fair Haven Community Health Care is looking for a Pediatric Care Coordinator to enhance patient navigation and access to care. Responsibilities include outreach to vulnerable patients, conducting needs assessments, and working closely with clinical teams. A candidate should have an Associate's degree in a health-related field or relevant experience, excellent computer skills, and experience with EPIC electronic health records is preferred.

Bilingual candidates in English and Spanish are highly desirable. This position provides crucial support within the healthcare system.
