Physical Therapist – Enfield, CT. FOX Rehabilitation is growing in Enfield and Windsor, CT. We are looking for licensed Physical Therapists to provide exceptional one-on-one care to older adults in private residences or Senior Living Communities. Our Geriatric House Calls™ model focuses on mobility, independence, and quality of life. Flexibility, autonomy, and meaningful patient outcomes are valued, with the ability to create and manage your own treatment schedule and build strong patient relationships.

Telehealth and other virtual care options may be incorporated to extend access and continuity of care.

• Provide one-on-one physical therapy to older adults wherever they live (private residences, senior living communities, independent living, assisted living, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.

• Full-time Physical Therapist – In-Home & Senior Living Care (salaried with benefits).
• Part-time Physical Therapist – In-Home & Senior Living Care (salaried with benefits eligibility based on minimum hours).
• PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit).

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

• Ability to meet the physical demands of in-home and community-based care.
• Medium physical effort; lifting 25–50 lbs occasionally, 10–25 lbs frequently, up to 10 lbs constantly.
• Perform CPR; assist with patient transfers and bed mobility.
• Repositioning, standing/walking for extended periods, stairs, and therapeutic activities.
• Driving to patient homes and senior living communities as part of the Geriatric House Calls model.
• Wearing PPE when required.

In Connecticut, the standard base pay range for a Full-Time role is $70,000 - $120,000 annually with an opportunity to earn more. Part-Time and FOX Flex (PRN) will be pro-rated.
